The decision to move into a skilled nursing facility for rehabilitation or long term care is never an easy decision for anyone. It is scary for the resident and their loved ones to have to make this transition. The role of our Social Services department is to help with the adjustment period, along with discharge planning, psychologist, and psycho social needs.

Gogebic Medical Care Facility has a trained social service professional. Chris Hoff can assist residents and their families daily with many issues and concerns. The main concern for the social workers at our Facility is to assure resident’s rights and preserve their quality of life. Our social worker also helps residents with their families cope with chronic, acute, or terminal illnesses and handle problems that may stand in the way or recovery or rehabilitation.

Discharge Planning is a large part of the every day business of social services. Planning for discharge begins the day a resident enters the facility. Our social worker is a part of an interdisciplinary team that will develop a discharge plan tailored to your individual needs, which may include: your medical condition, functional ability, financial constraints, and availability of family and community assistance. Some of the options that will be recommended may include assistance with arranging the necessary services to enable you to continue your recovery at home. Chris is also available to help you and your family after your discharge if a concern arises.

Some other areas of concern that social services deals with include: Completing Medical Assistance applications and/or re-certification; Executing Power of Attorney for Health Care Documents; Individual counseling for residents and/or families; Maintain contact with families; Assisting with Legal or financial matters along with a variety or other issues.

Our Social Service Department strives to provide the maximum amount of support needed by the resident and their family with the strictest of confidentiality.
